2. Order by Fax
Fax the order form to (818)767-6132. Fax ordering is available
24 hours a day, 7 days a week. This ordering option is for
credit card orders only. Please be sure to include the following
information:
email address, first and last name, address, city, state,
zip, daytime and evening phone numbers, credit card number,
name on card and expiration date, item name, item number,
color, size and quantity.
Please note that the minimum order amount is $50.

If you would rather pay by check or money order, please complete
the order form, and you may send your Money Order, Cashiers
check or Credit Card information to:
